The Loyalty card is valid at: Park Hospital 186, Park Road, Colombo ‐05, you are eligible for the discount for all the services offered by Park Hospital in the premises of the hospital.
The holder of the card will receive the discounts mentioned below
The card must be presented at Park Hospital in order to receive discounts.
The card cannot be used in conjunction with any other promotions or special offers.
The card offer is not valid for payments by credit card and only valid for cash or cheque payments.
Data Protection. The personal data you provide to us will be used solely for the purpose of administering the discount card scheme. Your personal data will not be passed onto any third party.
Park Hospital (Pvt) Ltd reserves the right to change the terms and conditions under which the Loyalty card is offered at anytime without prior notice.
Discounts for X‐ray will exclude reporting charges.
All discounts will be calculated after deducting the doctor’s charges if any.
The membership is granted for one year (12 calendar months). A nominal Membership fee of Rs. 2500/‐ will be charged at the time of issuing the card.
